ME3 Retrain Powers broken

At some point during the game I've unlocked all the squadmates powers. I switched to Reave when I got it, but next time I checked back on the console half the powers were gone. Only the most recent two I unlocked were there (from Omega DLC).

I just unlocked dominate and now the Omega DLC ones are gone as well.
