ME:L - Squad mates often stuck
Squad mates are unable to path around each other or NPCs on Eden Prime. It is very easy for them to get stuck.

This is an old bug from the old version. I figured they would've fixed this. It will happen throughout the entire game.
Maybe? I never had this issue at all playing the original, but it's real annoying right now. It seems to concentrate on main missions the most - Therum wasn't bad, but Noveria and the tunnels on Feros so far are real bad (I haven't finished Feros or done Virmire yet).
