@holger1405 I saw this yesterday, maybe it's related (at least worth a look)?:
it could very well be that in both cases flags are set wrong.
Did you saw other posts about this two issues?
I did not find any more topics about Zaeed or Miranda dying in ME3 despite being loyal.
- @holger1405 Hi having the same problem, I completed ME2 with 100 percent with full paragon and romanced Jack. I didn't romance Miranda but I did all of the requirements that should've kept her alive. Did all 3 meetings in the Citadel, warned her about KL, gave access to the specter terminal, read the KL dossier, and got full paragon bar. Did not have these kind of problems in the original ME3, didn't do anything different in the Legendary Edition.
