it could very well be that in both cases flags are set wrong.
Did you saw other posts about this two issues?
@holger1405 Hi having the same problem, I completed ME2 with 100 percent with full paragon and romanced Jack. I didn't romance Miranda but I did all of the requirements that should've kept her alive. Did all 3 meetings in the Citadel, warned her about KL, gave access to the specter terminal, read the KL dossier, and got full paragon bar. Did not have these kind of problems in the original ME3, didn't do anything different in the Legendary Edition.
What platform are you on?
I'm using the Xbox series S. Two other things I notice in ME3, Kelly Chambers and the comic. I saved Kelly during the suicide mission along with the rest of the crew, Dr. Chakwas is even in the Normandy. No sign of Kelly Chambers anywhere during ME3. Also the Genesis comic didn't show up at the start of ME3 which is weird cause usually there was an option to view it before the start opening cutscene.
@DanieruSempai This doesn't seem relevant to the original post, but Kelly only shows up in ME3 if you have dinner with her in ME2 (in addition to surviving, obviously). I'm on Xbox One and she has showed up as normal.
SpoilerAdditionally, if you're looking for her after completing Priority: The Citadel II, she will be missing if you hadn't talked to her yet in ME3 and told her to change her identity.
