Never been this angry at a game...
Never in my life have I come so close to losing my cool completely over a game. Never.
I'd written this game off a long time ago. Just too clunky compared to newer titles.

In the phase where you fight the boss, the characters staying behind have to add up to an adequate defensive score. Otherwise, characters start dying, with disloyal characters being prioritized.
An example of a perfect suicide mission run in your case (assuming only Tali is disloyal):
- Fire team leader: Garrus, Miranda or Jacob
- Tech expert: If you have the DLC, pick Kasumi. Tali and Legion are ineligible in your playthrough since they are not loyal.
- Crew escort: Someone with a low defensive score, like Mordin, Jack or Kasumi
- Biotic expert: Samara / Morinth, or Jack
- Final boss squad: Two loyal characters with a low defensive score, like Mordin, Jack and Kasumi
