Dr Chakwas is supposed to be recruited from Huerta memorial hospital. She definitely survived my me2 run-through. But I don't see her anywhere. I've almost finished me3, I have only the leviathan dlc ans the Cerberus base/priority earth left. Have been thorough about checking the citadel but haven't seen her anywhere.
Was Dr. Michel present at the hospital near the beginning of the game?
I think story-wise, Chakwas is supposed to join a different medical team unless you recruit her early on. It would make sense for her to not stay around in the hospital after a certain point, but I haven't tested that myself. The only instance where Chakwas is not available near the start is if the game considers her dead. I'm not aware of such an import bug, but it could be possible.
Do you have any old save files from earlier in the game to see if Chakwas was in the hospital?
Additional to @Fred_vdp, you must recruit her before "Priority: The Citadel II" after that mission she will be gone.
Chakwas was in Huerta memorial when you brought in Ashley/Kaidan and checked on them. If you accidentally recruited Dr Michel instead, I think Chakwas turns up as a war asset working on the crucible. If you didn't get to her before the Cerberus attack, I'm not sure what becomes of her.
