@purya_m I have seen this bug reported before in this exact way. It seems there is a bug for those who go to save the person at the bomb site, and the work around until a patch fixes it(hopefully) is to pick the other team mate to save at the Salarians. Honestly, I pick the Salarians every time so I haven't personally seen this bug, but there is a fairly long thread on this forum about this bug.
Was that on a specific platform?
I rescued Ash on the bomb side and had no problems whatsoever.
The Salarians and the Captain are saved either way if you destroyed the com tower, the flyers and the satellite uplink.

It seams that is is not platform related, so the repair might work. We see.
