@Exospector211 Yes happened to me 4 times already on various different missions in ME3, it is probably the buggiest title out of the trilogy.
From my observation this is caused by Liara's psy-hole-thing, doesn't excuse that it's happening but as a prevention measure you could avoid using her. Other then that, do manual saves often so if it happens you don't have to re-do 20-30 min of progress.
