While it is pretty common for MELE achievements to be bugged, it would help if you told us what you're actually doing to get this one. It might be just user error on your part.

The achievement description is not helpful at all, but basically you have to win a mirror match in the Armax Arena (Citadel DLC) while the difficulty is set to at least Normal.

    Thank you! I honestly swear my game was set to Normal.. apparently on Casual?

    I have done the mirror match on normal difficulty four times—the first two with Wrex as a teammate, then two more with Miranda and EDI. I restarted a load before the sliver strip DLC to do it again with Tali and Lara, but there is still no achievement.
