Can you first download the following drivers, specific to your setup:
http://support.amd.com/en-us/download/desktop?os=Windows+7+-+64
Then, use this uninstall utility to remove your current drivers:
http://support.amd.com/en-us/kb-articles/Pages/AMD-Clean-Uninstall-Utility.aspx
Then install the newly downloaded drivers from above.
Reboot and test your situation, then if the issue continues, please post your full Dxdiag report as detailed in the previous link.

I am still having the issue after the clean uninstall/reinstall. Attached is the requested DxDiag information. To supplement it, my motherboard is an ASUS SABERTOOTH 990FX R2.0

Do you happen to have custom firmware on the GPU or are you using any graphics injector like Sweet FX or similar?
- 9 years agoI do not. I do have three monitors hooked up - 2 samsung monitors and a cintiq drawing tablet - and I did a little reading and from what I gathered, there is a problem with this latest driver when running games with multiple monitors connected. However I still could not find a fix for it.

Hmm, it could be your Cintiq tablet is causing an issue. For testing purposes can you uninstall and disconnect that and let us know if there is any change please.
