The NFS errors in your Dxdiag refer to ntdll.dll which is a system file that is potentially corrupt.
Please run System File Checker to scan for and potentially fix any issues with your Windows system files: http://support.microsoft.com/kb/929833
Do post your results with that here, thanks.
I did, and it can't find anything out of the ordinary.

Can you right click the game tile in Origin and then click Repair.
Retry and let us know if there is any change, thanks.

Hello,
Sorry I should have said all the things I've tried during the week. I have:
1. Repaired mutiple times. Tried rebooting between repairs as well.
2. I have run a DISM /Online /Cleanup-Image /RestoreHealth command in administrator mode.
3. I have reinstalled the game.
4. I have tried running the game in solo mode.
5. Tuned down my graphics card.
6. Upgraded my drivers.
7. Played with various graphics settings.
It feels like I have done everything short of re-formatting my entire computer. The same error always comes up, the ntdll.dll one.

The following has a relatively low chance of working but is worth a shot:
Disconnect your secondary monitor, reboot and retry. Alternatively, make sure both monitors are running at the same resolution and refresh rate.
