I googled "form score" and it's pretty much a +/- and I do believe EA has it this way. So if you get scored on while on the ice your "form score" goes down and if you get a goal or teammates get a goal while on the ice your "form score" goes up. If you score a goal and have an assist and you get scored on while on the ice 3 times you get a - and your "form score? goes down. If you get a 0 on +/- the "form score" doesn't move.
So maybe this can explain to low form score if you have multiple points a game.
Simple solution is to change "form score" to +/-.
