I hope that is not the case, but definitely is a probability. I thought it was just by point production because I noticed the games where I had just 1 goal or assist it would always be 99 or 100 form score regardless on my coach grade. The games where I had A- coach feedback score and no points, I would have 20 form score.
One of the things that make plus minus a bad stat to go by is let's say you're a winger and player your position. Covering your defensive opponent at the blue line. Your Defense leaves man open back door and they score. Now you have "bad line defense" from the coach grade, despite you playing your role correctly, and your form score plumets. This has always been frustrating on previous BaP years, so if that is effecting form score too, it is a double whammy.

It doesn't even have to be while covering the defenseman. We can all attest to having those situations where the CPU makes a bad line change while the puck is in your zone, you step onto the ice, and end up with a goal against less than 3 seconds into your shift. Couple that with the ongoing issue of zero ice time periods as a fourth liner and it makes the form score a brutal roadblock.

They definitely need to change the "form score" because it's silly the way it is in the game, there are plenty of real life NHL players that have a lot of minuses in their seasons and they still have a lot of ice time. Connor Bedard is one of them he was a -80 in his career .
