Crazy Mode achievement glitch

My friends and I have escaped on crazy mode several times now and the game freezes or disconnects every signle time and we cant get the achievement. We have had all of us try hosting and we have played it spread over a few months and have the same problem every time regardless! Why is this a problem?!?!?!

This is on the 360 system
