Try pressing the plates in the order they describe. One of the posts has a diagram so you can be sure you're reading it correctly.
The code is obtained through the rotating (with a specific blank side) tiles found at the end of each colored portal. You need to input them in their exact positions that you found them (whether top then next one all down to the bottom) and type (boot,cartwheel,etc.) It is best to either write down the location of each tile or with modern technology take a screenshot with your game so you lest forget or need help putting in the code.
