Do you happen to have a Clip of this happening?
@iluminatethesky I don't have it. I also post a new topic regarding about the broken sound after the patch. You can imagine that if the game sound got broken then obviously the reaction sound doesn't work properly either.

Hey there! I haven't seen a whole lot of reports around this so far, do you have an external hard drive for your PS4 that's plugged in?
What type of audio device do you use (headset) and have you tried changing your audio settings on the PS4 to default?You said it started happening after the November patch, it's possible the patch was corrupted during the download and a reinstall might be able to fix this for you.
You could be right with it being a bug, but let's make sure technical issues are ruled out here first with these steps before we identify it as a bug since there's also not a whole lot of reporting around this at the moment. 👍
