But it has only occurred one time, and resolved itself correct?

Are you able to provide any further details?
How/where did you die? On stairs? Did you ragdoll like crazy? What did you die to? Did your body fall through the ground? Usually for death issue you can tell something is wrong as your body is already in a bad place, before you even get revived as it looks wrong.
How were you revived? A player/End of round/Bossmode?

5)end of round 10, as i was unrevivable via players due to the bug
6)as seen in the video, i was uncontollable, none of the powerups worked, even when i was alive, one of the players said "what happened here!?" and another said something like "am i seeing properly?", the fourth was micless. The first time i died was via 4 zombies, and halfway thru the ding motion, my character resumed its normal position, and floated, then, when it started working, it said you have been revived, like if the wave ends less than 20n seconds after you die, only it was over a minute I had been dead, and started playing like normal.
Ragdoll is when your body is flung around by physics, thrown into the air by an explosion/powerful hit or something. I watched the video yes, but what it doesn't show is how the issue occurred which is the important part.
When you first died, was your body floating while dead or after you've been revived? I assume the revive was from a player because you would have spawned elsewhere if you had timed out and revived at the end of the wave.
No ragdoll. The issue occured when I was killed by four surrounding zombies. While dead, I was unrivivable via player, and had no self revives. at the time of the revive, the other players were at the garden. I thought that I would have to but didn't as one player with a mic said "Hey! It wont let me revive you!".
