I've been able to finish the first single player campaign and started on Resurrection. Seems to be working fine now. I'll update if I run into this issue again.

I have this same bug:
I had no luck with the clean install/Conflicting files (non found), everything was as it should be.(Also I do not use surge protectors and power supply is bang on I know this as I'm a Veteran Military sparky/engineer)
So let's kick off your questions for more details for you buddy.
(If anyone else is experiencing the same problem for the last mission Hard crash at the start of the last mission or on its loading screen, enter your answers)

I just experienced this crash while playing through the Campaign mode. As other people have already stated, I was seeing intermittent lag during previous levels but nothing too bad. Was able to play through all the levels until I get to the Lando Mission 'Cache Grab' on Sullust. The game freezes and if you hit the Home button the Xbox goes back to the dashboard briefly then reboots itself. It restarts itself automatically.

What mission is it crashing in? What are you exactly doing before it crashes?
The Lando mission on Sullust. Right at the very beginning level as Lando and Shriv are moving down the path to the installation, if I crouch and try to sneak up on the stormtroopers eventually the game freezes as the stormtrooper dialog begins, the sound goes haywire and begins rotating around each channel on my 5.1. home theater system. Occasionally bits of dialog will play but the game remains frozen. If I then hit the Home button to go back to the dashboard, the screen goes dark for several seconds, the dashboard appears for a few very brief seconds then the Xbox reboots completely.

Yes. I just reinstalled and started playing the Campaign from the beginning. The other levels played normally with no major issues. Some lag spikes but they were brief and the system recovered. Every time I try to start the Lando mission on Sullust the game eventually freezes and I get the Xbox reboot.

I had the exact same problem and contacted support. They told me to do the following and seems to have worked.
XBOX One:
CLEAR ALTERNATE MAC ADDRESS
1. Go to Settings > Network Settings > Advanced Settings > Alternative Mac Address > Clear
2. After this shut down the console, disconnect the power cord from the back of the Xbox One and wait two minutes.
3. Once the two minutes have passed, plug the power back in and wait for the power bricks light to go from White to Orange.
4. Once the brick is Orange, start your console.
