I'm having similar issues on PS4. Sometimes the grenade doesn't do any damage, sometimes it bounces off walls and does nothing. Other times a grenade doesn't even get throw, particularly if you throw one directly after coming out sentry mode. The animation occurs but nothing gets thrown.
