This BUG:
Since Launch. I have no hopes this will ever get fixed.
Yeah it has happen since launch.....but within the past month or so it is getting to the point where it occurs every game now instead of every 50-100 games. Sometimes it even happens multiple times in a game to the same person or a few people. It's becoming worse.....I think that the servers need a rest or something too to help.
