3 Databank entries won’t scan on Bogano and one chest on Ilum won’t register

Summarize your bug Entry 13 for “The Researcher”, entry 5 for “Remnants of a Lost Civilization”, and entry 2 for “The Creature” are either not scannable or not registering in the databank, and the chest in the “Jedi Temple” will not register on the holomap even though I’ve already opened it.
How often does the bug occur? Every time (100%)
Steps: How can we find the bug ourselves? Follow the bug summary. When you get to the locations for the databank entries they will not scan, and the chest on Ilum is under water and I have gone to that chest multiple times and it still doesn’t register.
What happens when the bug occurs? I cannot complet the game 100%
What do you expect to see? Complete databank and 100% completion for the game.

This game is fantastic, but these few small bugs are bothering me enough that I created an EA account to complain because I want the 100% completion on the game. Please help!
