After troubleshooting this further I found that Xbox 360 elite controller is detected but non of my joysticks. Found the culprit though. You need to turn off "Generic Gamepad Configuration Support" in Steam > Settings > Controller > General Controller Settings.
Here:
Ensure you UNTICK Generic Gamepad Configuration Support, then exit out and reconnect your HOTAS. Restart the game and go to options -> controls and scroll down to the bottom of the list and you should see your HOTAS listed there like here:
Sorry had to repost this as the first post failed. Anyway, this bug should not have gotten past QA but anyway, it did and this fix solved my issue. Hope it helps you and others out.

Oh I see...you are referring to the pictures on page 1 that has absolutely nothing to do with my issue that I posted. Maybe try reading the whole thread 🙂
- 5 years ago@SuperChadMonkey Hi. I see you're having trouble with your Hotas. You can check out my threads on getting it to work. I won't repeat all the details here - I'd just be duplicating it but here's the lowdown.
SW:S can only see 4 HID devices and it filters them based on if they have an X axis - if they don't - it won't show them and you can't use them.
Some HID devices break SW:S - i.e. Thrustmaster Cougar MFD's (#2 specifically).
You have to unplug all usb hid devices and controllers from your PC except stick, throttle and rudders.
If your rudders don't have an x axis, you have to get their firmware updated to fake it - see my VKB fix.
You can (as I did) buy a 10port switchable usb hub to make this simpler when moving from flight sims to SW:S.
Also, as I just said to someone else, if it grabs your throttle as the first device it can break your mouse and never let you get it back. Way to get around that is hold down the right mouse button and change the first flight device to something other than your throttle and then it works.

That level of fix is insane, and I am not sure it applies to me either. I only have 2 USB devices plugged in (Mouse and KB) and my stick/throttle is only 1 other USB connection as well (T.Flight HOTAS 4). I have never tried to add any other devices, and from the sound of it, extra items make it worse.
How on earth can a AAA title be released with this level of user manipulation to get their primary advertised control system to function as designed? I have no other issues with MS Flight simulator or DCS world, which means this is their game specifically.
At this point I have let my pro EA subscription expire and have no intention of starting it back up until this issue is fixed. I have started in on Red Dead 2 to fill my time.
In short-I'm out until they fix the game. Make no mistake, the game is broken, not the users.
