No Story Completion Awards/Trophies, Missing Medals (PS4)

What were you doing when the bug occurred? Step by Step detail is helpful. Completed story missions on pilot difficulty
Where did this issue occur? Please include menu name or in game location. After story completion

How often does the bug occur? Every time (100%)
How severe is your issue? Can play with mild frustration
What happens when the bug occurs? Did not receive trophies or awards for completing story on pilot difficulty. Also all medals earned during story missions seem to have vanished.
What do you expect to see? 2 trophies -The Galaxy's Finest (Story Mode) & The Galaxy's Finest (Pilot). Unlocked ship customization options. Medals appearing when I browse story missions.
Which ship were you playing? If not applicable, just answer "no" or "none." None
What components were you using? None
Which customizations (if any) were you using? None

I completed the story on pilot difficulty and expected a couple of trophies to pop but was surprised when they didn't.  I also checked the ship customisation options that were supposed to unlock and found that they hadn't.

Finally, after reading on this forum about missing medals in story missions I checked mine and found them all gone.

    Same issue for me with pilot mode on PC.

    Same on Steam PC when completing game in Veteran. I tried reflying a mission I had accidentally finished on Ace thinking having the one done on a different difficulty was causing the issue. The medals I earned on that run (but not the previous one) showed up after finishing the mission, then disappeared again after playing a fleet battle match.

    All my medals are gone too. I had completed the game on Ace only to find it didn't count, so I went back and beat 8 of the earlier missions again on Ace, with medals appearing in 13/14 missions. Upon reopening the game, all my medals are gone.

    I've just finished story mode on highest level of difficulty and... i didn't get any trophy and game lost every my medal from missions 1-5. I finished story mode second time, first time i didn't get any trophy too, but i thought that it happened because i changed level of difficulty on higher during it... This game is a joke. I am trophy hunter - just give me back my trophies or money. Natahriel from ps4.

    Same problem on Xbox One. I just finished the campaign and didn’t get any achievements for finishing on Pilot difficulty. Also, all medal progress has been wiped for every mission but the last. That’s super annoying.

    @xdragon2688 Same with me. beat on ace, started again but no medals saved, did first mission again (to get all medals), and then played a online match. all medals disappeared again.

    Best solution, redo the game on story mode, only playing campaign you'll keep all medals off you don't switch modes
