Occasionally, when fighting The Second Sister on the lower levels of Zeffo, she will jump to do a special move...and...just never comes back down again.
She just jumps clear up through the architecture and vanishes.
There is no way to progress past this, as you have to beat her enough (but not actually win) for BD-1 to save you. Otherwise you're just stuck there, no way to go back, no way to go forward.
Fortunately, the Meditation Circle is nearby. My solution was to quit the game and restart.
It worked.
I'd advise against walking off the ledge to diminish your health until you respawn, that seems to be a risky option for the bugs in this game.
She's tough, so if the first few times she whomps you, on your respawn it will have saved the XP she's carrying from your last attempt, so this isn't a breaking bug if she suddenly disappears (but it comes pretty close, still should have been fixed in testing, or should be fixed in a patch).
