Not able to enter phon'qi caverns after dying to bosses

Summarize your bug Unable to enter phon'qi caverns after dying to the bosses. The entrance to the cave is unavailable, and I can't fast travel to the meditation point.
How often does the bug occur? Every time (100%)
Steps: How can we find the bug ourselves? Enter the cave and die to the bosses at the end of the cave without resting at the meditation point in the cave. You respawn outside the cave and you're unable to enter the cave again both from the entrance and fast travel.
What happens when the bug occurs? When I get to the entrance of the cave I suspect the metal plate that's originally blocking the path is still there, but I can't see it so therefore I cannot remove it. Cal just ends up walking into the entrance like there's a wall blocking the path that I can't see nor remove.
What do you expect to see? I expect to be able to re-enter the phon'qi caverns after dying to the bosses.

When you enter the cave you need to use force to remove a metal plate blocking the entrance to the path. after exploring the cave I died to the 3 bosses at the end of the cave and respawned on another meditation point outside the cave.

Since I respawned outside the cave I'm not sure if I actually "rested" at the meditation point in the cave, but it does show up on my mini-map. however it doesn't show up in my fast travel map, so I'm unable to fast travel to this part of the map.
