One thing I can see is Windows 10 is still running 14393 RS1 is this intentional ?
try first disabling the Intel video card it doesn't need to stay enabled Card name: Intel(R) HD Graphics.
Then try the new AMD drivers at this link
Regarding the Windows 10 part: i wouldnt call it intentional, as i didnt manually stop windows from upgrading anything it tried to
So far disabling the Intel Card and updating the AMD Drivers seemed to do the trick though (which kinda surprised me since the the system kept telling me the drivers were up to date)

Yours appears is version 14393 which is on 1607 build by the looks (Link to Windows 10 Builds) , latest is the 1709 build and is worthwhile for the fixes.
