I have done that multiple times. Just forgot to add it to that list. sucks i have to do it every time i turn my computer on though.
But it will still occur even in debug mode
Dxdiag directrions are at this link if you can do that would be great
Also press the windows key and type "reliability" and open the reliability moinitor and see what errors are being recorded

I can see the swtor crashes in the dxdiag as well , they dont help though
Given this is windows 10 , noting multi monitor there was a Win 10 issue previously however the error reporting for that issue is different to what is displayed for you.
But given there is no real indication , and it is something to try , see if the following makes any change.
Initially - in the ingame setting place grass to 0
if it persists also go back to a single monitor with grass on 0
this was the workaround for that Windows 10 issue but it was recently confined I thought to certain SLI setups not multi monitor
but it is something different to try

Also try putting shaders on very low , there is thread that has appeared recently and sounds similar in what is happening in this thread that appeared today
Nvidia did fix a shadows issue recently in the driver for swtor as well , would need to look up which version but was in this half of the year.
When you say tried older drivers , what version did you go back too ?
Just trying to get as much information as possible at present.
