Most common reason for this is they are too high a level
What level are you and what level are the targets anything over +3 will be heading to where they cannot be hit.
But i still could attack with skills that go through dodging and others, but the other problem is its not that i keep missing this happens most of the time where sometimes they keep getting hit while sometimes they can only get hit a couple of times *All of the enemies were in the same Story Mission*
have tried seeing if your weapon needs repairing at a vendor ? it maybe broken
