Did you ever had Shae Vizla as a companion? (From Eternal Throne pre-order or through the SWTORCompanionChaos), you can write an email to support@swtor.com , to get your companion back.
I did have her as a companion, and she is usable on my other characters, but I only got her recently along with Nico Okarr. As soon as I started Eternal Throne with this character I lost her.
Are you able to summon her if you go to the fleet? Some story chapters will lock you out of specific companions until you complete the story chapter and this may be the case here. Try going to the fleet and seeing if you can summon her.
- Negatory. Cannot summon in fleet. But I agree, it must have something to do with the story chapters. Im gonna power through em so I can get her back.

Having any of these missions/chapters active in your log will prevent you from summoning Shae, due to her involvement in the story:
- The Enemy Within
- Blood Hunt
- Torch's Flame
- Chapter XIV: Mandalore's Revenge
