Stuck on old wounds questline, cant progress

I get to the quest "ongoing threats" , after completing this quest, it just give me the same quest again and again, i cant get to the next quest which is "secrets of the shrine"

I have tried reseting the questline, same issue once i get too " ongoing threats"

I apologize for the late response. Wanted to follow up if you were still experiencing this issue? The only thing I could find on this issue was to make sure you don't have any older conflicting missions in your log or to go to like the fleet and reset the mission then go to back.
