Swtor fallen empire companion bug Lana

I play the fallen empire expansion and when I play chapter 4 gravestone Lana is missing and dont spawn so i cant progress when they said to speak with her . I have try many thing like resetting my cu...

Try going to the fleet after you've cleared out any mission logs then right click your character portrait and choose reset active phases.  Then reset the mission and see if that works.

@Odithebodi I can see you're back on Chapter III now, I'd recommend playing through the chapters until you hit a block again, and then wait for your in-game ticket to be picked up, it's in the correct queue waiting to be handled.
