Conquest Hard Mode

For conquest hard mode, it states you have to beat normal mode before attempting hard mode. Is that just one time or you have to go through Normal each time before getting to do hard?

Do you need to replay tier 6 of the Sith Triumvirate raid every time before you launch Heroic?
