It's applied every turn to every enemy in a loop so long as DN is alive. So the simplest way to analyze it is that save for cleansing and resists you have 20% less Health to burn through. So its like a constant expose which heals will not replenish since Max health is capped.
Yup its a pretty good debuff to have constantly applied, but it won't be very effective against a team with tenacity up.
