PSA: Last Tier Revan Shard missing [BUG REPORT]

Been waiting Months to get Revan and when I finally unlock him, the game resets and now I can't unlock or complete the final tier again. Has this happened to anyone else???

What did you guys do to break Revan? Haven't seen this happen to anyone in my guild yet.
