Has anyone seen it attack twice in a ship battle? It claims to be 40% but it seems highly unlikely to me, as I've never seen it happen a single time.

It doesn't animate the attack multiple times; instead, it just does multiple hits (like Rey's multi-hit attack). And yes, it triggers pretty frequently for me.
