Asajj's offense does not stack while she is dead, but what was previously stacked is not removed when she dies.
So she retains massive offense boost through death but doesn’t continue to increase the boost while actively dead. Also, if she is “partially revived” from Talzin lead and that attack/assist kills the target, she is currently alive and will stack offense on that kill in addition to staying revived.
If she and a second Nightsister are temporarily revived via Talzins Leadership + a double acolyte attack, does Asajj's unique "see" the other nightsister die? (Or since they die simultaneously, does it not add?)
The response from Kozi would seem to indicate that it does not since it also implies she doesn't see herself die even though the ability doesn't specify "other" ally. If both happen at the same time, she wouldn't see the other one die either.
It seems like the stack order goes: character dies and is off the field, then the damage boost gets added to the stack, at which point Asaji is not on the field for the ability to resolve and increase her damage, etc.
