Talzin and AV lead?
Dug around a bit haven't seen this anywhere but if you run AV lead Talzins lead also works? How long has this been going on, only seems to work in GA?

Most likey Daka revived her and you weren't paying attention. Otherwise this would have been reported by more than just you and 3 of your guildmates.
