Refund on the web store for something I never refunded?

Earlier today I saw that someone posted on Reddit about having something they bought on the web store refunded to them when they didn’t issue a refund. Now that has happened to me when I never issued a refunded for 2 items that I bought recently on the web store. Is this a new error or bug? I have heard that issuing a refund could get you banned but I never issued one for theses purchases what is going on with the web store? Are the accounts that this is affecting safe?
