Is this the game skin or a default replacement? Did you have default replacements, but remove them? I know sometimes this can happen if you install default replacements or remove them, but it doesn't update correctly. Even if that's the game skin, I do have some suggestions that may help.
Take your Sim to a mirror and change their eyebrows. If that doesn't do it, go into your Sims 2 folder and delete cache files and thumbnails. If that doesn't fix it, I suggest getting the ffs lot debugger (aka batbox). Then try Fix- tans and burns.
It's the game skin! I haven't used any mods/ccs in my game so this is how it was when i downloaded it. Eyebrow trick, removing cache, thumbnails (also even removing logs and repairing the game once again) didn't work either :(
Is the only way to fix it by downloading mods?
I'm honestly not sure. It's possible, but it's also possible that won't even work. You might be able to try also changing hair or clothes. I can't think of anything else, short of trying default skin replacements. Other than ignoring it and hoping it self corrects while you play.
But I always recommend the ffs lot debugger to everyone, even people who otherwise don't use mods or cc. It was created specifically to address many of the problems that come up in the game that we've always had. It was originally created in 2005 and has been kept updated. It's so ubiquitous within the Sims community that it has it's own wiki page. I've been using it myself for over a decade.
