Sim appeared on lot during phone call?

A male sim called a female sim, his brother joined the call and after talking for a while suddenly the phones hang up (there was not any animation showing them hanging up) and later I find the female sim trapped in the "foundation" under the elevator? 

So to be clear, she lives on another lot and somehow appeared on the current lot I was playing.
