I find that defaults & genetics can be finicky. Here is a couple of things I'd try ( & learnt):
I occasionally find that rebel Sim that refuses to use my defaults (it is usually a townie). Sometimes I find going into CAS changing the Sim in someway helps to get the defaults working. This is assuming that see a change in CAS.
Trying putting them into the overrides folder. The overrrides folder is favoured over the mods/package folder & clear your cache especially world cache & simCompositorCache.
Have you tried just loading the game with the default skin? Then if that works trying it with just the eyes.
Also if you have merged your content together, maybe you should re-download everything in that file from scratch and re-merge. I had problems with getting a taxi default changed from one to another. I had to re-download everything in that file to swap to the one that I wanted. To this day I don't know what the problem was because both files looked identical but the new file finally changed the default.
I've discovered through testing out a lot of different genetics that this game can hold onto these things in its memory, even after deleting all I've mentioned. I was recently debating about changing skins & downloaded a few. I found that a skin that I tried, kept showing up on the first time I tried changing things in CAS. I didn't like it but my game seemed to.
Just try playing, sometimes these things take a while to kick in.
