You first have to install the Sim through your launcher.
Then you start the game, go to create a Sim. The Sim you downloaded can be found under premade sims. Choose the sim, edit to your liking (if you want) and then place the sim like you would with one you've created yourself.

Not to sound dumb but were do you find the button for pre made sim?
I found it. If any one else has had this trouble Go to C.A.S, Click on Pre-Made Sims (The box with Sim shapes on it, bottom left.) then scroll till you find the one u want use.
