Check if there are new graphic drivers for your computer. yours are about 3 years old.
I'm rather bad at hard ware diagnosis and I know almost nothing about AMD processors but it seems to me, that CPU is not really made for gaming. My suggetion to you is to post your DxDiag in the technical part of the official forum where there are people that know about this kind of stuff and maybe can give you a better opinion on your computer.
