@starbelly7 Please post a screenshot.
Please also let me know what happens if you move the Saves folder out of Documents > Electronic Arts > The Sims 3. This will prevent the game from reading your save, and I'd like to know whether you can start a new save or not. If the folder copies rather than moving, or you get an error when trying to move it, please let me know that as well.

Now I can only choose Sunvalley. I had removed the saves folder each time I installed a new world. It let me install two. Then I put them back and started up the game again and could only choose Sunvalley. The Installed Worlds folder is empty.

@starbelly7 In your first screenshot, New Game is grayed out because you've already selected it, or it was auto-selected. Once you choose a world, eg. Isla Paradiso as is showing in the list, you can click the check mark to load that new save. To load an existing save, you'd need to select it from the icons below the view of IP, although your save looks like it might not be accessible in that screenshot.
In your more recent screenshot, are you sure you've enabled all packs in the launcher's Expansions & Stuff tab? If they're not loading, that would explain why only Sunset Valley is an option. If they're already checked, uncheck all of them and re-check them.
The saves you made may not be playable in this screenshot because the worlds they take place in haven't loaded.
As for InstalledWorlds, that should be empty unless you've installed Store or custom worlds through the launcher. It doesn't hold any data for Sunset Valley or the worlds that came with expansion packs.
