Well, your CPU is on the low end and your graphic card is below specs.

Have you tried if lowering the ingame graphic settings and turning off mirror reflections help ? Did you try DEP ? Have you tried running the game without custom content or mods ?
Yeah, I've tried Dep and no custom content but I'll try lowering my graphics to see if that helps.
