@ILN22 Hi I've had some issues like this in CAS myself. I'm on Xbox Series X.
So are you using Play with Genetics to create the family member? That's how I do it so that the Sim is created from the original Sim?
If I don't use Play with Genetics and create the parent that way, if the Sim has been in my game for a while I usually cannot add a relationship between an existing Sim and a new household member in CAS. We recently raised it in feedback and it seems to be an issue on console.
If nothing is working this way for you have you tried it with a new household to test if it is working in your game at all?
On Xbox Series X I was just able to create a sister this way.
Also if you have most packs it's quicker to list what you don't have on the bug report but it's helpful to know that info so that we know what packs may be a factor. So if you could add that information by editing your bug report in the top right hand corner that would be great to know.
Hi! Thanks for responding. I did try the Play with Genetics feature and once it generated, it said “household member” instead of “Mother”, even though I selected to play my sims mother. I haven’t tried to test it on a new Sim because I really wanted my favorite sim to have a mother. I should probably mention as well that this particular sim had a mother, father and siblings but I noticed one day that everyone gone from her genealogy so I tried to re add the mom and sisters for starters to the household (sacrificing their lives and storylines) and it just never worked. The only relationship options for the mom are the two I mentioned and the only relationship options for the sisters are now “household member” and “mother”. It’s ssoo backwards. I can definitely go back and add all my in owned packs to the report, thanks for that tip.

I don't play on a console, but on a PC/Mac you can't add a family relationship to a Sim that previously had one. For example: they had a Mother who was culled by the game. Even if the family member/s have disappeared from the genealogy panel, the game still counts the disappeared Sim/s as the Parent/Sibling.

Awwww man, well thanks for that info. I guess I’ll just forfeit the old legacy and start building a new one with this generation and her children and so on.

@ILN22 in that case it's not a bug if they had previously already had a mother. As @DizzyDee-K rightly says they can't add another mother as far as I know.
So it's working as intended. I'll mark this as Solved.
You could always just recreate the family but then you lose your progress.
If the Sim had been teen age or younger they could have had a new adoptive parent though. I've done that before and selected Adopt as Care Dependent. But with young adult or older they can't do that. I've also added unofficial step parents for Young Adult Sims. I know it's a step parent even though the game doesn't as yet have the title of step parent which is a shame.
