With this latest update on/around 4/10/21 my sims have begun autonomous hand washing again. I know this because, whenever there's a new update, I remove all mods and clear my cache, then start a brand new save to check that everything is working ok.  I'm not sure if there are any updated mods to correct the issue because it's getting annoying.

@ERM0621 Same here. May 12, 2021 and they still obsessively try to wash their hands all the time.

    @astronuggie Yes. Same here. However, weirdly the house I'm currently playing...it's only ONE of the sims doing this.

      I have seen it too lately, Sims wash their hands several times per day even when almost at max hygiene.
