I don't think this is a bug. If it's correct their 'spouses' also react to catching them in the act if that happens.
I think it's something of a simulation of an affair.

I don't know. At the Sims 3 at least they needed to break up first.
The only strange thing I can see from the screenshots is that they get double icons that don't update.
For example christiaan has the 'tortelduifjes' icon twice (the lovebirds icon for those who don't get dutch), just two in different stages. The same with the other screenshot. I don't think they should have 3 different types of relationships when one is simply the next level of one of the other icons.
As for the ´Sim collecting harem thing´....it´s not a bug. They changed a few things from sims 3, this was one of them. So you no longer need to break up with someone in order to have an affair behind their back......which was, to be honest, quite silly in Sims 3
