Do they actually not witness it (meaning do they not show any reaction) or does it just not register ?
And just for completion sake: Are you using any mods ?

I don't use mods and 3 sims who died of old age have not been "witnessed" either.
No I can't get them to witness it, they stand right next to it, walk around, even get tense because of a fire. I even tried pleading with the Grim Reeper and it failed but still not complete.
No mods.

I'm having the same problem... Witnessed three deaths already, and no reactions. (and no aspiration fullfilled).
My game has also fulfilled the 'reach lvl 8 in crime job' also if I'm only on level 4 instead, I don't know if it's related.
Also, most people I've met and a boyfriend too don't show up in my people list, I can't call them as they are not there. I don't know if they are still in the game.And last, I keep 'meeting for the first time' people I'm sure I've already met before.
Everything is happening after last update, never had any of those problems before.
