The take on health I'd like would be some realistic (maybe trait-based) over random.
Make it so sims with Loves the Outdoors have immunity to allergies and lower chance to become ill but the chance grows when they are inside for a long time. Fat/lazy people with a higher chance of digestive issues. Lazy/Hot-headed with a higher chance of heart attack. Sims with high-level fitness would have a lower chance to be ill. Gardening skill reducing the chance for allergies. Possibly connect it with Seasons/weather so sims getting wet in rain or cold in winter would have higher short-term chance to get ill. Squeamish sims could have panic strokes when they encounter someone who's ill.
Introducing vaccines might be a possibility as well.
Maybe have all the aspects (short-term and permanent) combined into some kind of meter (similar to parenthood traits) that'd show the likelihood of getting ill for each sim.
